I need a little more lift than my jack plate can gice me. Has anyone heard of or used a 5 inch vertical extension plate sold by Mid-Tn marine or Southern charm.com?

Get the 4" from cmc. It's fairly cheap and works well. It will give you 4" of setback and 5 " of vert. I had one on my Allison.

CMC = cook manufacturing.
I think the website is www.cookmfg.com not sure though. They make jackplates etc..
RD <--- doesn't know anything about outboards so don't ask me any questions. Just happened to know that

I found vertical lift plates but i didnt see any with set back in them.

It wont hurt it any. Look under acceseries or just call them and ask for a 4" set back with a 5" vertical travel. I just looked at mine but no model number on it.

TD are you talking about a new jack plate? If so mine has 5 inches of lift and 1 inch on the mounts but it aint enough.

Yes, put them togetherand you'll have all the adjustment you need.

My Detweiller plate as 7" of lift from bottom to top. Why do you need more than 5" of lift though? Perhaps the jackplate is mounted too low on the transom or can you move the motor up on the jackplate?

My jack plate dosnt go high enough i have it all the way up and in the top bolt holes. I need about a inch and a half to get even with the pad.

I think you need the bottom holes to raise the motor up.
