Forgot to lower the landing gear... (http://www.zianet.com/tedmorris/dg/bombers4.html)
Not sure I believe they forgot....there should be a gear/flap warning horn I would think.......and a checklist

Yes, there is, both. Perhaps one didn't work (malfunction), and the other wasn't used due to complacency and fatigue (not excuses, reasons for the end of their flying careers). Can you cay "Wing Flight Safety Officer" now mr pilot?

For all that happened, it appears to be in good shape.

thats one way to lose your job. but for a belly landing that plane stayed very well intact. and was still able to roll.

I read the incident report in Avaition Week and Space Technology....the gear was called out on the checklist, yet wasn't lowered. The aircraft flew for nine more minutes with a big red light shining on the panel.
Amazingly, the aircraft doesn't have a ground proximity warning system that alerts the crew if the gear is down below a certain altitude.
