FWIW, I smoked for 25 years. Tried quitting a few times, and wasn't ready (obviously). About 5 years ago, I decided enough is enough, used the patch, and quit. Did I fall back a couple times? Yes. What was the secret to me quitting? 2 things. I noticed that I always lit up at the same time (or circumstance). Get in the car, lite up. Work on the boat or rod, have one lit. THAT was the hardest thing to break. The physical addiction wasn't nearly as bad.
The other thing that REALLY sealed it was, one day I cheated and lit one up, after having quit for a month or two. Took a couple drags, and instantly got light headed, nauseaous, and started to sweat. Took another deep drag and spent the next 45 minutes puking my guts out. No more urge whatsoever. (kinda like that night ya drank too much Southern Comfort, and now you can't even SAY Southern Comfort without getting sick to your stomach!)
Hope you're ready to quit and get over the hump.
