I wonder if we can respond more than once. The state isn't organized enough to figure that out are they?
I faxed in one for myself, one for my wife, and one each for my 3 kids. The one from the kids was more about their safety concerns than about the money. I also got my Mom, sister and 2 brothers to send one in along with others they got to do the same.
